I understand that District's Public Information Officer/Public Educator may photograph the Class for publicity purposes and, by checking the box in the photo release section of this registration form, I consent to photography of me and my child at the Class, and I consent to the District's use and publication of said photos for publicity purposes.

I understand that participation in the Class can lead to serious physical injuries. I hereby assume the risk that I/my child may suffer injury as a result of his or her participation in the class. I agree for myself/my child and on behalf of my/my child's heirs, representatives, successors and assigns that the District, including its officers, employees, and agents, will not be liable for any damages or injuries I/my child may suffer while participating in the Class unless caused by gross negligence of the District.

I agree to indemnify and hold harmless the District, and each of its officers, employees, and agents from any claim, action or proceeding brought in any jurisdictions against any of the foregoing persons resulting from any action, whether by my/my child's negligence or otherwise, while participating in the Class. This indemnification shall include, but not be limited to, any damages, fees, costs, liabilities, or expense incurred by the District in connection with such proceeding. I shall further indemnify the District and its officers, employees, and agents for all costs, fees and damages which the District may incur in enforcing this indemnification provision.
